Why do western people think Ahmadinejad is in charge of Iran?

Most of them have never heard of Khamenei who is the real leader. This tactics used by Iran is very similar to a figure head tactic where you put one person as the scapegoat to take all the heat from the people while the people behind the scapegoat are doing all the damage.
